■ 고시 개정 전체내용 1. 허가사항 범위 내에서 아래와 같은 기준으로 투여 시 요양급여를 인정하며, 동 인정기준 이외에는 약값 전액을 환자가 부담토록 함. - 아 래 - 가. 폐색전증 환자에게 투여한 경우 1) 동 약제를 반드시 투여하여야 하는 임상적 기준(Criteria) 가) 저혈압 또는 산소투여로 호전되지 않는 저산소혈증(hypoxemia) 나) 심초음파도상 우심실 이상운동증(dyskinesia) 다) extensive ileofemoral thrombosis ※ 위 Criteria 중 1가지만 해당되어도 투여가능 2) 투여시기, 기간 및 용량 가) 투여시기： 증상발현 후 2주 이내 나) 기간 및 용량： 허가사항대로 100㎎을 2시간에 걸쳐 정맥주입 나. 급성 혈전성 관상동맥폐색증의 경우 발작 후 6시간 이내에 투여한 경우에 한하여 1치료기간 중 최대 100mg 범위에서 요양급여를 인정함. 다. 급성 허혈뇌졸중에 증상 발현 후 4.5시간 이내에 투여한 경우에 한하여 0.9mg/kg 범위에서 요양급여를 인정함. 2. 허가사항 범위를 초과하여 아래와 같은 기준으로 투여 시 요양급여를 인정함. - 아 래 - 가. 말초혈관폐색증 나. 급성 뇌동맥혈전증의 혈전용해술 다. 두개강내 혈종의 용해목적으로 카테터를 통해 주입하는 경우 라. 폐렴주위 흉수(Parapneumonic pleural effusion)에 흉관삽관(Thoracotomy catheter)을 하였으나 중격(Septum)이 형성되어 흉수(Pleural fluid)가 잘 배액 되지 않는 경우 마. 중심혈관 카테터가 막혔을 경우 헤파린 주입(Flushing)을 반복 실시하여 효과가 없는 경우 바. 인공판막혈전증(Prosthetic valve thrombosis)의 정맥 내 혈전용해요법 사. 연령 관련 황반변성에 의한 망막하 출혈 1) 투여대상: 황반중심와를 침범한 황반하 출혈이 발생한 경우 14일 이내 2) 제외대상: 출혈 경향 전신질환이 있거나 출혈 경향 유발 약제를 복용 중인 경우 3) 투여방법: 최대 50μg를 유리체강내 또는 망막하 주사(안구당 1회) ■ 고시 개정 고시번호(시행일자) 고시 제2025-102호(2025.7.1.) ■ 고시 개정 사유 ○교과서, 가이드라인, 임상연구문헌, 전문가 의견 등을 고려하여, 연령 관련 황반변성에 의한 망막하 출혈 환자에 대해 급여 확대함. ■ 관련근거 · Ryan’s Retina, seventh edition. 2023 · Harrison's Principles of Internal Medicine, 21e, 2022 · Ophthalmology, sixth Edition, 2023 (Yanoff & Duker) · The Royal College of Ophthalmologists.
