■ 고시 개정 전체내용 허가사항 범위 내에서 아래와 같은 기준으로 투여 시 요양급여를 인정하며, 동 인정기준 이외에는 약값 전액을 환자가 부담토록 함. - 아 래 - 가. 투여대상 : 동종 조혈모세포 이식수술(HSCT)을 받은 거대세포바이러스(CMV)-혈청양성[R+] 성인으로 투여 시작 전 5일 이내 혈청 CMV 검사(PCR)에서 음성으로 확인된 환자 나. 투여방법 : 이식 당일 및 이식 후 28일 이내 투여를 시작하며, 투여기간은 다음과 같음 - 다 음 - 1) 이식 후 100일까지 투여 인정함. 2) 고위험 환자에 해당될 경우 200일까지 투여 인정함. ○ 고위험 환자: HLA(Human leukocyte antigen) 불일치·반일치 이식, 제대혈 이식, ATG(Anti-thymocyte globulin) 투여 이식, 이식편대숙주질환(GVHD)으로 치료중인 환자, 고용량 스테로이드 투여 환자 등 다. 투여중지 기준 1) CMV 질환이 발생한 경우 2) 선제치료(Preemptive therapy)가 필요한 경우: 거대세포바이러스 항원 혈증(CMV antigenemia) (혈액내 CMV 항원 검출) 양성소견이나, 혈액으로 실시한 CMV-PCR검사의 양성소견을 토대로 선제치료가 필요하다고 진료담당의사가 판단한 경우 ■ 고시 개정 고시번호(시행일자) 고시 제2025-88호(2025.6.1.) ■ 고시 개정 사유 ○교과서, 가이드라인, 임상논문, 학회(전문가) 의견 등을 참조하여 고위험환자의 경우 200일까지 급여기준 확대 및 투여중지 기준을 보완함. ■ 관련근거 · NCCN Guidelines, Prevention and Treatment of Cancer-Related Infections.2024. · 대한이식학회, Prevention of Cytomegalovirus Infection in Solid Organ Transplant Recipients: Guidelines by the Korean Society of Infectious Diseases and the Korean Society for Transplantation.2024. · Domenico Russo et al.
