■ 고시 개정 전체내용 1. 허가사항 범위 내에서 투여 시 요양급여를 인정하며, 허가사항 중 “거대세포바이러스(CMV; Cytomegalovirus)질환 감염 위험이 있는 장기(臟器)이식 환자의 CMV 질환의 예방” 에는 아래와 같은 기준으로 투여 시 요양급여를 인정하며, 동 기준 이외에는 약값 전액을 환자가 부담토록 함. - 아 래- 가. 선제치료(Preemptive therapy) : 장기이식 후 거대세포바이러스 항원 혈증(CMV antigenemia) (혈액내 CMV 항원 검출) 양성소견이나, 혈액으로 실시한 CMV-PCR검사의 양성소견을 토대로 선제치료가 필요하다고 진료담당의사가 판단한 경우 나.예방적 투여(Universal prophylaxis therapy 또는 Prophylactic therapy) 1) 장기이식술 전 공여자가 거대세포바이러스 혈청면역글로불린G(CMV serum IgG) 양성이고 수혜자가 CMV serum IgG 음성인 경우 2)장기이식술 후 면역억제제로 ALG(Antilymnphocyte Globulin), ATG(Antithymocyte Globulin), Orthoclone OKT3(올소클론)를 투여한 경우 2. 허가사항 범위를 초과하여 성인의 급성망막괴사증후군(ARN) 또는 거대세포바이러스 망막염(CMVR)에 아래와 같은 기준으로 투여 시 요양급여를 인정함. - 아 래 - 1) 투여 대상 가) 전신 항바이러스제제 투여에 반응이 없거나 부작용 등으로 투여할 수 없는 경우 또는 나) 망막병변의 진행이 빠르고 시력을 위협하는 경우 2)투여방법:유리체강 내 주사(2mg/0.1ml) 3) 투여간격: 1주 1-3회 투여 이후 환자상태에 따라 감량가능 ■ 고시 개정 고시번호(시행일자) 고시 제2025-88호(2025.6.1.) ■ 고시 개정 사유 ○교과서, 가이드라인, 임상논문, 학회(전문가) 의견 등을 참조하여 Letermovir 경구제 및 주사제 급여기준이 변경됨에 따라 선제치료 관련 문구를 동일하게 함. ■ 관련근거 · NCCN Guidelines, Prevention and Treatment of Cancer-Related Infections.2024. · 대한이식학회, Prevention of Cytomegalovirus Infection in Solid Organ Transplant Recipients: Guidelines by the Korean Society of Infectious Diseases and the Korean Society for Transplantation.2024. · Domenico Russo et al.
